CPE at The Center for Integral Spiritual Formation

CISF does not charge an application fee at this time. Students accepted into the program must submit a $300 non-refundable deposit within two weeks of acceptance to reserve their seat in the cohort. This deposit is applied toward tuition. The remaining balance may be broken evenly into monthly payments due on the 1st day of each month in which the term runs.

I understand that the ACPE Educator may use my written evaluation, their written evaluation of me, and other clinical materials for the purposes of certification, peer review, accreditation, or ACPE-approved research.

I understand that identifying information will be removed from written materials, but live or recorded observations may still include my likeness.

I understand that these materials may be viewed by colleagues evaluating the educator’s development or the center’s compliance with ACPE standards, and may be retained indefinitely.

I understand I may revoke this consent in writing, but doing so will make me ineligible to complete the unit or receive credit. Materials created before revocation may still be used.
